Summary: The PATH Lead collaborates closely with the Director of Client Services to identify, develop, and implement innovative programming that ensures our clients achieve their established goals.  This position is key in maintaining authorizations, scheduling, and general staff oversight.  This position actively works toward enhancing the organization’s mission, vision, values, and goals by assisting the CEO, Human Resource Manager, and Directors, ensuring a value-based, employee-oriented, collaborative, high-performance team culture that is focused on innovative client solutions.

Essential Functions, Duties, and Responsibilities: Duties may be modified or additional duties assigned.

  1. Makes routine follow-up calls and emails to ensure proper authorizations are in place. Ensure authorizations are accurate and do not expire.
  2. Be the main point of contact for clients, families, and county Case Workers.
  3. Serves as the front-line contact for staff to address client behavior and develop plans for response and behavioral care planning.
  4. Works with the Director of Client Services and Human Resources in hiring, onboarding, and training new Associates.
  5. Work closely with the Senior Associate to coordinate staff and client schedules to include student transportation weekly.
  6. Works with the Director and the Senior Associate to plan, research, organize, and develop a well-rounded and innovative program that meets individual client needs and county requirements for summer camp, after-school camp, and school break camps.
  7. Addresses staff concerns or performance concerns and provides guidance as needed, in coordination with the Director of Client Services and HR.
  8. Provides on-the-floor support to the daily programming by leading activities, groups, or outings in a person-centered manner.
  9. Assists Director of Client Services in approving PTO requests.
  10. Drives bus or van to transport individuals to and from PATH and for Special events as backup.
  11. Maintains all safety protocols and ensures the client area is clean and sanitized per policy and procedure.
  12. Provides input on the program budget, keeping expense reports up to date and delivered to the Director of Client Services at the end of each month.
  13. Communicates to the Director of Client Services staffing-related needs based on established guidelines in relation to client census.

Mechanical or Technical Skills, Qualifications, Experience:

Some advanced coursework in Human Services, Social Work, and Special Education or related; Associate degree or Special Education or Human Services preferred.  At least 3 years’ experience working or volunteering with individuals with physical and developmental disabilities or in case management. 

 

  • High School Diploma or equivalent with some advanced coursework in Human Services, Social Work, and Special Education or related; Associate Degree or Special Education or Human Services or Healthcare related preferred.
  • At least 3 years’ experience working or volunteering with individuals with physical and developmental disabilities.
  • Must have a good understanding of Client Rights, confidentiality, and become familiar with county guidelines to assist the organization in maintaining compliance.
  • Must be creative and innovative with excellent artistic abilities, such as painting, drawing, musical, drama, etc.
  • Attention to detail, excellent written and verbal skills are required.
  • Deal effectively with people at all levels of responsibility, including clients, staff, volunteers, Case Workers, family, and others involved with our clients.
  • Must be an independent worker and able to respond to a variety of situations calmly and professionally.
  • Must have the ability to communicate and interact with disabled youth in a proactive, positive manner.
  • Must have an outgoing personality and be spontaneous when working with clients to create an energetic/happy environment.
  • Must complete CPR/First Aid/AED within 6 months of employment.
