SUMMARY: The Day Service Associate Position is responsible for providing individualized, person-centered care to ensure our clients experience a safe, engaging, and fulfilling day. An Associate will facilitate age and interest appropriate programs that promote social, emotional, cognitive, and physical development. This role includes providing personal care as outlined in individual care plans to support optimal daily functioning with respect and dignity.

The Day Service Associate position will be expected to work in close collaboration with all team members to ensure consistent, high-quality support while fostering a positive team environment focused on continuous improvement and shared team and organizational growth. This position may also be trained in other departments as needed due to client census.

Essential Functions, Duties and Responsibilities: Duties may be modified or additional duties assigned

· Works with Day Service Lead to ensure clients participate in daily/weekly/monthly schedule of programs, outings, events and activities based on needs, abilities and interest to clients; develops, implements, and evaluates programing that impacts client lifestyle, promotes positive behaviors, stimulation and meaningful socialization on a 1:1 basis or in groups.

· Assist Day Service Lead with researching and planning of monthly activity schedules; utilizing creative thinking to benefit the clients served.

· Maintains all safety protocols and ensures client area is clean and sanitized per policy and procedure.

· May be asked to lead Life Skills Classes as needed. Transport Clients for Services and Activities as scheduled.

· Assists with personal careas needed; to maintain an optimal level of functioning, cleanliness, eating/meal preparation, toileting, ambulation etc. Ensure all medications are given properly according to the medication protocol, ensure safety during lunch and snack breaks.

· Application and maintenance of orthotics as prescribed.

· Works closely with SRDC Case Manager and Day Service Lead to report on the status of clients based on their care plans.

· Works with volunteers when scheduled for activities and events.

· Maintain and demonstrate the Mission, Vision, and goals of SRDC at a high level.

· Participate in continual professional development.

Mechanical, Technical Skills, Knowledge/Education, Qualifications, Licensure/Certification, Experience

  • Outgoing and friendly to all.
  • Ability to maintain confidential information, display good judgment, and respond to emergencies or in difficult situations.
  • Deal effectively with people at all levels of responsibility including visitors, clients, staff and volunteers.
  • Ability to attend to multiple tasks throughout the workday, coordinating multiple priorities and schedules accurately.
  • CNA certified or PCW, Special Education Teacher/Assistant preferred and/or working with adults and with disabilities or experience with a senior population serving those with memory loss related to Alzheimer’s or other forms of dementia; minimum 2 years’ experience preferred.
  • Minimum High School Diploma or Equivalent.
  • Valid Driver’s License and Excellent Driving Record with 2 years’ experience,
  • Minimum Eighteen (18) years of age.
  • Safety Conscious, Dependable, Good decision-making skills.
  • Ability to respond effectively during medical or emergency situations.
  • Ability to organize, prioritize and manage tasks without direct supervision.
  • Excellent oral and written communication skills; able to effectively communicate with clients, co-workers, volunteers, and management.
  • Deal effectively with people at all levels of responsibility including participants, staff, volunteers, etc.
  • Patient, self-confident and sensitive to individual needs.
  • Able to adapt easily to frequent change.
  • Excellent customer service and listening skills.
  • Team Player - work with others to execute the policies and goals of the organization.
  • CPR/AED and First Aide Certified or within 6 months of hire.
